Please arrive 15 minutes prior to your appointment to complete your new patient form, alternatively this can be completed in advance below.

New Patient Deposit
Over the past year we have had a large number of patients fail to attend their first appointment with the surgery after booking in with us. These late cancellations and no-shows have forced us to request a deposit from all new patients to secure an initial booking. The deposit paid is deducted from the cost of the first appointment. Should the appointment not proceed, the deposit will be returned to the patient as long as notification of cancellation is provided by the day before the appointment.
New Patient Forms
New patients may be asked to make a long appointment in order to allow enough time to update past medical history as well as address the presenting complaint.
We can organise for your past medical records to be transferred from your previous doctor.
To make your first visit as smooth as possible, we ask new patients to complete a registration form before their appointment.
